    Circuit Court, Small Claims — Eviction · revised 03/26 · printed form (we type onto it)

    Get the return date, time and place from the clerk (or as your county's local rule says) and write them in the Summons box; tick whether the tenant must appear and/or file a written answer. If the facts do not fit, attach a page and tick "See attached for additional information".
