Write the court name the way the court uses it, e.g. Buffalo City Court or Suffolk County District Court, 2nd District. List the rent owed month by month in the table on page 1 yourself. The Good Cause Eviction Law notice (pages 3-8) must be completed by you: tick whether the unit is covered and why. Sign the verification in front of a notary.

UCS-LTN1 — Notice of Petition – Nonpayment
City/District/Town/Village Courts — Summary Proceedings · statewide (outside NYC) · fillable PDF
Write the court name the way the court uses it, e.g. Buffalo City Court or Suffolk County District Court, 2nd District. Attach this notice to the nonpayment petition (UCS-LTN2). The clerk fills in the court date, time and room and signs it unless your lawyer does.
