The Self Help Center says these forms are only for landlords who are individuals: a business, corporation or LLC must be represented by a North Dakota lawyer. Three-day notice of intention to evict (N.D.C.C. 47-32-01/02). Sign and date it, serve it, and keep a copy for the complaint.

Attach the lease and the notice of intention to evict as exhibits and write their letters on the form. Paragraph 7 (damages to the property) and the lease's late-fee terms must be completed by hand.
