  • Complaint

    CN 11252 (Appendix XI-X) — Verified Complaint – Residential Landlord Tenant (Nonpayment of Rent)

    Superior Court, Law Division, Special Civil Part — Landlord/Tenant · fillable PDF

    Mandatory statewide form. Complete by hand what the wizard does not ask: owner of record and ownership type (items 1-6), landlord registration (7-9), rent control, the filing fee and the totals in items 19-23, the holdover cause-of-action code (24) and the landlord verification on page 5. Late fees and other charges are listed as "additional rent" only if your written lease calls them additional rent — remove them otherwise. A corporation or LLC must be represented by an attorney.

  • Summons

    CN 10822 (Appendix XI-B) — Landlord/Tenant Summons (Appendix XI-B)

    Special Civil Part — Landlord/Tenant · revised Revised effective 09/01/2025 · fillable PDF

    The court fills in the docket number, trial date and time, the Special Civil Part phone and the local legal-services, bar association and welfare numbers, and the clerk signs the summons.
