Hawaii has no single statewide eviction complaint form. Here is what the courts there publish, and how to prepare your papers.
No statewide eviction complaint form in Hawaii
Judiciary publishes circuit-specific (not single statewide) district-court summary possession forms; each circuit numbers its own (1DC, 2DC, 3DC, 5DC).
What this means for you: ask the clerk of the court where the property is located which eviction complaint and summons it uses (many courts publish their own), or have an attorney prepare the pleadings.
