  • Complaint

    Form 1A — Verified Complaint for Possession of Real Property — Nonpayment of Rent (Residential)

    Superior Court, Civil Division — Landlord and Tenant Branch · District of Columbia · fillable PDF

    The complaint must be signed under oath before a notary or the clerk. Fill in the Rental Accommodations Division registration/exemption number, the Basic Business License details, any other pending cases, and the months of rent owed (from / to) yourself.

  • Complaint

    Form 1B — Complaint — Violation of Obligations of Tenancy or Other Grounds for Eviction

    Superior Court — L&T Branch · District of Columbia · fillable PDF

    The complaint must be signed under oath before a notary or the clerk. Cite the lease paragraph or Housing Code section the tenant violated and complete section 3(A)(2)(b)-(c) yourself.

    Form 1C — Complaint for Nonpayment of Rent and Notice to Quit

    The complaint must be signed under oath before a notary or the clerk. Fill in the Rental Accommodations Division registration/exemption number, the Basic Business License details, any other pending cases, and the months of rent owed (from / to) yourself. Use Form 1C only when you claim unpaid rent and another ground together.
